Upper Blepharoplasty

Lower eyelid aging is primarily a volume and structural issue. What is eyelid surgery?

Eyelid surgery, or blepharoplasty, involves removing excess fat and skin around the eyes. Depending on the patient's particular issues and anatomy, the upper and lower eyelids can be done together (double eyelid surgery) or individually. The goal of the procedure is to create a refreshed, youthful appearance that looks natural.

Instead of asking "what have you had done?", your friends and family will comment on how happy, restful, and refreshed you look. An upper blepharoplasty is a relatively simple, minimally-invasive procedure that can yield dramatic, life-changing results in the right patient.

The Blepharoplasty Procedure

Precision techniques for an undetectable outcome.

Your blepharoplasty procedure is performed in our private, fully accredited surgical facility in Bryn Mawr. Depending on whether you are undergoing an isolated eyelid surgery or combining it with a deep plane facelift, it can be performed under local anesthesia with twilight sedation or general anesthesia.

  • The Upper Incision: For the upper eyelids, your surgeon places a delicate incision perfectly hidden within the natural crease of the eye. Through this access point, precise amounts of skin and muscle are addressed.
  • The Lower Incision: For the lower eyelids, we frequently utilize a transconjunctival approach. The incision is hidden entirely on the inside of the lower eyelid, leaving zero visible scarring on the outside of the face. If a small amount of excess lower eyelid skin needs to be pinched and removed, a micro-incision is placed just millimeters below the lower lash line, where it heals virtually invisibly.

The procedure typically takes one to two hours, and you will return home the same day to begin your recovery.

What to expect during your healing timeline.

A smooth blepharoplasty recovery requires patience, but the physical downtime is highly manageable. Because Dr. Stark and Dr. Campbell utilize preservation techniques that respect the vascular network of the eye, surgical trauma and bruising are minimized.

  • The First 48 Hours: You will experience mild to moderate swelling and bruising around the eyes. Discomfort is generally minimal and easily managed with over-the-counter pain medication and cold compresses. You will need to keep your head elevated, even while sleeping.
  • Days 3 to 7: Swelling begins to subside rapidly. If non-dissolvable sutures were used, they will be removed at our office around day five or six.
  • The Return to Normalcy: Most of our professional patients feel comfortable returning to the office, participating in video calls, and running errands within 7 to 10 days. Any residual bruising can easily be concealed with light makeup or glasses.

Blepharoplasty Results

Longevity and structural integrity.

Your final blepharoplasty results will fully emerge over the course of a few months as the last fractions of micro-swelling resolve and the incisions fade entirely into the natural contours of your eyes.

Because we focus on deep-tissue repositioning rather than just skin removal, the outcomes are exceptionally long-lasting. While your face will naturally continue to age, the heavy hooding and pronounced under-eye bags rarely return. Upper
blepharoplasty

Upper blepharoplasty is a very straightforward procedure and can be performed right in the office at StarkMD Plastic Surgery and Aesthetic Center under local anesthesia. After making one incision in the crease of the eyelid, Dr. Stark removes excess skin before carefully and methodically closing the incision. His skillful suturing technique as well as the location of the incision allow for the eyelid surgery scar to be completely hidden. The sutures are removed five days post-operatively.

Lower blepharoplasty

Lower blepharoplasty, the eyelid surgery for under the eye, is slightly more involved due to the anatomy and the usual need to remove excess fat, in addition to just skin. For this reason, general anesthesia is necessary for this procedure.

To perform lower eyelid surgery Philadelphia plastic surgeon, Dr. Stark makes an incision under the lower lash line or within the eye along the lower eye-lid. Through this incision, Dr. Stark is able to remove the excess tissue carefully, maintaining the underlying muscular support as well as the shape of the eye. Bruising and swelling are typically more severe after this type of surgery than from an upper blepharoplasty but tend to resolve within one to two weeks.

In addition to the excess fat that accumulates under the eye, there is often hollowing that occurs at the junction between the lower eyelid and cheek. A lower blepharoplasty is often combined with fat grafting and lasering to help smooth the skin and blend the appearance of this groove.

Will a blepharoplasty change the shape of my eyes?

No. This is a common fear stemming from outdated surgical techniques. Our preservation-first approach ensures that the fundamental shape and character of your eyes remain completely intact. Are blepharoplasty scars visible?

No. Plastic surgeons are experts in scar camouflage. Upper eyelid incisions are hidden deep within the natural crease of the lid, making them invisible when your eyes are open. Lower eyelid incisions are typically made inside the eyelid itself (transconjunctival) or concealed directly beneath the lower lash line. Can a blepharoplasty be combined with other facial procedures?
